ADMISSION POLICY
A. Admission Requirements
1. Old MDPS Students
The MDPS F138 (report card) is a requirement for enrolment. The report card verifies that a student is free of any obligation in the previous school year, and therefore is qualified to enroll for the current year.
2. New Students
a. A new student who wants to enroll at MDPS should meet the following requirements in accordance with the DepEd and MDPS regulations:
• Age Requirement:
Nursery 1 - 3 years old
Nursery 2 - 4 years old
Kinder - 5 years old
Grade 1 - 6 years old
Grade 11 - 17-18 years old
• Report Card/certified true copy of the latest grades with a general average of 80% or above and no grades below 80% or failing grades in any subject
• Conduct grade not lower than S (85%-89%) and has no disciplinary case
• Copy of Philippine Statistics Authority (PSA) Birth Certificate
• Copy of Baptismal Certificate
• Certification of Good Moral Character/ Letter of Recommendation from the Principal/Guidance Counselor of previous school
• Medical Certificate (if your child has any medical issues)
• Two (2) recent photos (1x1) with name written at the back
• Accomplished MDPS Student Information Sheet
• Must pass the entrance examination (written and oral)
• Reservation fee of Php3000 (for qualified applicants only – non-refundable)
• All documents must be placed in a big brown envelope
b. A qualified transferee from Kinder to SHS (Senior High School) should present his/her report card bearing the signature of an authorized official from the school last attended certifying his/her eligibility for admission to the next level. A student who is unable to present his/her report card upon enrollment should fill out a waiver form.
c. Report cards that are stamped “Cancelled” or which show signs of alterations and erasures are not valid for admission. Any other credential presented for admission should be referred to the Principal for approval.
d. Placement of transferees from foreign countries seeking admission to MDPS
MDPS Admission Committee determines the appropriate elementary or secondary level in which to place the transferee/s (DECS Order No. 67 s. 1993 which are as follows:
a. Those who have completed the Sixth or Seventh Grade abroad are eligible for admission to Grade 7; however, graduates of a five-year elementary curriculum are eligible only for admission to Grade 6.
b. Those who have completed Eight Grade abroad are eligible for Grade 9, but have to take Filipino 7 and Social Studies 7 (Philippine History and Government)
c. Those who have completed the 9th Grade abroad are eligible for Grade 9, but shall take Filipino 7 and Social Studies 7, and take Filipino 8 in summer before enrolling for Grade 10.
Such prerequisite requirements shall have to be completed before the start of the academic year in schools offering summer classes.
B. Enrollment Procedures
Step 1
The enrollee is given a registration form in three kinds: registrar’s copy, treasurer’s copy and parent’s to be filled out completely. Accuracy of information is a requirement, particularly on names, home address and contact numbers.
Step 2
The enrolling student will submit the completed form to the enrolling personnel for assessment of payment.
Discounts:
• First honors transfer students for grade 1 to Grade 11 are given 50% discount on tuition fees upon submission of certification of the aforesaid award and other pertinent requirements.
• The fourth child student of four (4) siblings studying at MDPS receives 50% discount on tuition fees.
• Old students awarded first and second honors receive 50% and 25% discounts on tuition fee respectively.
Step 3
The student number is entered to the computer

3. Press “Enroll a student” (New Student or Old Student)
4. Fill out information needed to process enrollment.
5. Upload requirements:
• Report Card,
• PSA Birth Certificate
• Certificate of Good Moral Character
• 2 by 2 ID picture
• ESC Certificate / Voucher Certificate (for new students only)
6. Attest information.
7. Sign out.
